If you suffer or are at risk for diabetes and hypertension, Saint Peter’s University Hospital can help. If you are 18 years of age or
older, you can receive high quality care to better manage these chronic diseases — even if you have no insurance or not enough
of it to cover long-termmedical expenses.
In one convenient location at The Diabetes and Hypertension Center at Saint Peter’s Family Health Center, you can benefit from
comprehensive care provided by physicians, nurses, a nutritionist and a social worker, all experienced in helping you to better
manage your diabetes and hypertension.
In addition to primary care and follow-up care, we offer:

Education on managing medications and living with chronic disease
■
Nutrition counseling
■
Foot, skin and eye care
■
Support groups and information about community resources
■
Extended hours on Wednesdays until 7 pm and Saturdays from 8:30 am - 12:30 pm.
Our goal is to help you make healthy lifestyle changes so that you can succeed in managing your diabetes and hypertension,
